The Lakewood Service League accepts new member applications beginning April 1st and ending August 30th of each year.

Membership is finalized with a first year dues payment of $100 which can be paid via credit card after submitting the form below.
Please choose "Provisional" Membership Level if this is your first time applying to join the League. If you are a current member and renewing your membership, then please use the Membership Renewal Form which can be found in the Member Login Portal.
I understand that my participation with and membership (at any level) in the Lakewood Service League (“LSL”) is voluntary. In consideration for my membership in LSL, I release and forever discharge and hold harmless LSL, its officers, directors, members, successors and assigns from any and all liability, claims and demands whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my participation in: volunteer activities (whether specifically for LSL or for an organization that LSL supports), attendance of LSL meetings and any other LSL events (collectively, the “Activities”), whether caused by negligence of LSL or its officers, directors, members, employees, or agents or otherwise. I expressly and specifically assume the risk of injury or harm from the Activities and release LSL from all liability for injury, illness, death, or property damage resulting from the Activities. I understand that this release is intended to be as broad as allowed under the laws of the State of Texas.
I grant and convey unto LSL all right, title, and interest in any and all photographic images and video or audio recordings of me made by LSL during the Activities (the “Images”), including, but not limited to, any royalties, proceeds, or other benefit derived from the Images. I understand that LSL may use the Images in all promotional material, including social media, and give my permission for such use.
